Travel Registered Nurse Case Management Job
Travel RN Case Manager assignment in Vermont for home health and hospice services. 13-week contract starting 08/31/2026.
About the role
This travel role supports patients within a community-based care model and collaborates with an interdisciplinary team to manage care plans and transitions in a home healthcare and hospice environment.
Responsibilities
- Screen, evaluate, re-evaluate, and treat patients; coordinate and communicate treatment activities with the interdisciplinary team and the patient.
- Develop a care plan in collaboration with the patient’s physician, the patient, and family or primary caregivers at admission; recertify the plan as appropriate.
- Coordinate the patient’s care plan through interdisciplinary communication and case conferences; communicate discharges in a timely manner to other disciplines.
- Provide ongoing assessment of clinical, functional, and emotional status to identify problems and progress toward goals; document evaluations, goals, and objectives.
- Teach the patient and caregiver self-care techniques; provide medication, diet, and other instructions as ordered by the physician; offer health counseling opportunities to patients and caregivers.
- Communicate significant changes to the patient’s physician and VNH colleagues; ensure timely documentation and follow-up actions.
Requirements
- Active RN license in Vermont and New Hampshire, or an unencumbered compact license valid in both states.
- Minimum 2 years of Home Health experience.
- Graduate of an accredited school of nursing.
- Ability to work in a home health/hospice setting and participate in a travel/field-based role.
- Strong assessment, communication, and care coordination skills with the interdisciplinary team.
Pay
Weekly pay ranges from $2,215 to $2,373 with guaranteed 32 hours per week.
