Responsibilities

• Perform comprehensive, value-based health evaluations in members' homes within assigned regions.
• Conduct head-to-toe assessments, review medical histories, reconcile medications, and identify care gaps.
• Utilize technology and analytics tools to document findings accurately in the EHR.
• Support risk adjustment efforts and educate patients and caregivers about health management.
• Communicate key insights to health plans and primary care teams to enhance outcomes and reduce costs.

Qualifications

• Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience.

Skills

• Strong assessment, documentation, and communication skills.
• Ability to travel within assigned regions and work independently in patients' homes.
• Familiarity with electronic health records and risk adjustment processes.
