Travel Nurse Practitioner or Physician Assistant

Role: In-Home Traveler Nurse Practitioner or Physician Assistant Responsibilities Conduct comprehensive health evaluations for Medicare and vulnerable populations in patients' homes.Perform head-to-toe assessments, review medical histories, and reconcile medications.Identify care gaps using advanced technology platforms.Collaborate with an interdisciplinary team to document findings in the EHR.Support care coordination and provide patient education to enhance value-based care. Qualifications & Skills Current Nurse Practitioner or Physician Assistant licensure.Strong assessment, review, and documentation skills.Ability to work independently with autonomy in the field.Excellent communication and collaboration abilities.Comfort with travel within assigned region.
