About the role

Responsibilities
- Perform head-to-toe assessments and review medical histories
- Reconcile medications and identify care gaps using technology platforms
- Collaborate with interdisciplinary teams to document findings in the EHR
- Support care coordination and provide patient education to promote value-based care
Qualifications & Skills
- Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience
- Strong assessment, documentation, and communication skills
- Ability to work independently with autonomy in the field
- Comfort with using technology for documentation and care management
Key Duties
- Travel within assigned regions to conduct in-home health evaluations
