Travel Licensed Practical Nurse Long Term Care Job

Local 12-week contract assignment in Bala Cynwyd, PA starting 08/26/2026 under a Local Contract (Nursing) program in a home healthcare setting.

Responsibilities

• Deliver skilled LPN nursing care during home visits, with a focus on wound care management and documentation
• Assess patient condition, monitor vital signs, administer prescribed medications, and implement plan of care under supervision
• Perform wound care procedures, dressing changes, and routine clinical tasks in accordance with clinical policies
• Maintain accurate and timely charting in the electronic medical record; communicate updates to the RN supervisor and care team
• Educate patients and families on care plans, medication administration, safety, and self-management strategies
• Coordinate with physicians, therapists, and other providers to ensure continuity of care and adherence to care plans

Requirements

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (or eligible for licensure in PA)
• Minimum 2 years of home care experience
• Proficiency in wound care required
• Ability to work in a home healthcare setting and cover patient visits in 19104 and 19139 zip codes
• Comply with agency policies, documentation standards, and patient safety guidelines

Pay

Weekly pay range: $1,270–$1,317 with guaranteed 40 hours per week.

Schedule

5x8 days (8:00 AM–4:30 PM) for 12 weeks.
