About the role
Provide end-of-life care in patients' homes or hospice settings, ensuring holistic, dignified support for patients and their families.
Responsibilities
- Assess patient needs and develop individualized care plans
- Administer pain management and symptom control
- Educate families on care techniques and emotional support
- Monitor patient conditions and adjust care as needed
- Collaborate with interdisciplinary teams (physicians, social workers, aides)
- Maintain accurate documentation and adhere to safety protocols
- Provide emotional support to patients and families
Requirements
- Valid RN license
- At least one year of recent RN experience
- Minimum six months of hospice or palliative care experience
- BLS certification

- Strong communication and interpersonal effectiveness

- Ability to work under pressure in emotionally challenging situations
