Role: Transplant Coordinator RN Responsibilities Support complex pre- and post-transplant patient care in a leading academic health system.Guide patients and families through the transplant process, including evaluation, surgery preparation, and long-term follow-up.Perform detailed assessments, coordinate testing, and manage scheduling.Educate patients on medications and self-care practices.Monitor for potential complications and ensure safe, timely, and compliant care.Collaborate closely with surgeons, physicians, pharmacists, and social workers.Contribute to quality initiatives and uphold clinical excellence, ensuring compliance with UNOS and regulatory standards. Qualifications & Skills Registered Nurse licensure with relevant transplant care experience preferred.Strong assessment, coordination, and patient education skills.Ability to work collaboratively within multidisciplinary teams.Knowledge of transplant regulations, UNOS standards, and quality improvement practices.
