About the role
The transplant coordinator is a Registered Nurse (RN) responsible for managing care for transplant patients, living donors, and their families. They serve as a key member of the multidisciplinary team, facilitating the entire transplantation process through individualized education, support, and coordination of clinical, surgical, financial, psychosocial, nutritional, and pharmaceutical needs.
Responsibilities
- Advocate for patients throughout the transplantation process.
- Ensure compliance with regulatory standards.
- Document care accurately and comprehensively.
- Participate in quality improvement and performance improvement activities.
- Engage in patient care planning and program development to support patient-centered care.
Requirements
- Associate's or Bachelor's degree in Nursing.
- Active RN licensure (multi-state or single-state).
- Basic Life Support (BLS) certification.
- Minimum of 3 years of nursing experience.
