About the Role
Coordinate care for transplant patients, living donors, and families across all phases of transplantation. Provide individualized education, support, and facilitate communication within a multidisciplinary team. Assess medical, surgical, psychosocial, nutritional, and pharmaceutical needs while advocating for patients' best interests.

- Coordinate care for transplant patients, living donors, and families across all phases of transplantation.
- Provide individualized education, support, and facilitate communication within a multidisciplinary team.
- Assess medical, surgical, psychosocial, nutritional, and pharmaceutical needs; advocate for patients' best interests.
- Manage documentation adhering to policies and regulations, including timely updates in electronic health records.
- Participate in quality improvement and compliance activities related to transplantation and donation programs.
- Perform other assigned duties, ensuring adherence to organizational policies and standards.
Requirements
- Minimum 3 years of nursing experience.
- Associates or Bachelor's degree in Nursing preferred.
- Required certifications include BLS and RN licensure (multi-state or single state).
- Strong computer skills, excellent organization, and communication abilities.
