About the Role

Coordinate care for transplant patients, living donors, and families across all phases of transplantation. Provide individualized education, support, and facilitate communication within a multidisciplinary team. Assess medical, surgical, psychosocial, nutritional, and pharmaceutical needs while advocating for patients' best interests.

  • Provide individualized education, support, and facilitate communication within a multidisciplinary team.
  • Assess medical, surgical, psychosocial, nutritional, and pharmaceutical needs; advocate for patients' best interests.
  • Manage documentation adhering to policies and regulations, including timely updates in electronic health records.
  • Participate in quality improvement and compliance activities related to transplantation and donation programs.
  • Perform other assigned duties, ensuring adherence to organizational policies and standards.

Requirements

  • Minimum 3 years of nursing experience.
  • Associates or Bachelor's degree in Nursing preferred.
  • Required certifications include BLS and RN licensure (multi-state or single state).
  • Strong computer skills, excellent organization, and communication abilities.
