Transplant Coordinator RN

Responsibilities

• Coordinate care for transplant patients, living donors, and families across all phases of transplantation.
• Provide individualized education, support, and facilitate communication within a multidisciplinary team.
• Manage clinical aspects including patient evaluation, care planning, and performance improvement.
• Advocate for patients, ensuring appropriate transplantation or donation options and delivering patient-centered care.
• Maintain accurate documentation, update medical records, and ensure compliance with regulatory standards.
• Participate in quality monitoring and process improvement activities.
• Collaborate with external providers and ensure timely communication and documentation.

Qualifications & Skills

• Associates or Bachelor's degree in Nursing; RN license (multi-state or single-state)
• Minimum of 3 years nursing experience
• Required certifications: BLS and RN licensure
• Proficiency in computer use, excellent organizational and communication skills
