Transplant Coordinator RN

About the Role

Coordinate care for transplant patients, living donors, and families across all phases of transplantation. Provide individualized education, support, and facilitate communication within a multidisciplinary team.

Responsibilities

• Manage clinical aspects including patient evaluation, preparation, and ongoing care.
• Advocate for patients to ensure appropriate transplantation or donation options.
• Document care accurately, maintain compliance with policies, and update medical records timely.
• Participate in quality improvement initiatives and ensure adherence to regulatory standards.

Requirements

• Associate's or Bachelor's degree in Nursing.
• RN license (multi-state or single-state).
• Minimum of 3 years nursing experience.
• Certifications: BLS and RN licensure required upon hire.
• Strong organizational, communication, and computer skills.
