Transplant Coordinator RN

Role: Transplant Coordinator (RN) Responsibilities Coordinate care for transplant patients, living donors, and families across all phases of transplantation.Provide individualized education, support, and facilitate communication among multidisciplinary team members.Manage clinical aspects including patient care coordination, education, and performance improvement initiatives.Advocate for patients, ensuring appropriate transplantation or donation options, and deliver patient-centered care.Oversee documentation in compliance with policies, HIPAA regulations, and regulatory standards.Participate in quality monitoring, process improvements, and ensure adherence to transplant regulations.Collaborate with external providers and maintain accurate, timely medical records. Qualifications & Skills: Associates or Bachelor's Degree in Nursing (preferred).RN license (multi-state or single-state).Basic Life Support (BLS) certification.Minimum of 3 years nursing experience.Strong computer literacy, organization, and communication skills.
