Therapist - Housing Stability
About the role
Population served: Adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder, who may be experiencing housing instability or are recently housed after experiencing homelessness.
Responsibilities
- Office and field based services with transportation of clients using agency or personal vehicles, as appropriate
- Work with clients with active mental health symptoms and within their treatment preferences
- Meet clients where they are in the community including private homes or outdoors promoting confidentiality in all settings
- Demonstrate comfort and flexibility while working with clients in difficult life circumstances, which may include individuals living in substandard conditions
- Assess and diagnose clients presenting for services to determine eligibility to CCBHC services and housing stability team
- Refer client to most appropriate outpatient program according to assessment and diagnostic results, within or outside CCBHC
- Obtain consent for treatment from all responsible persons
- Establish appropriate and productive therapeutic alliances with the client(s) and caretakers, as applicable
- Complete safety assessments to include risk of violence towards self and others. Complete safety plans and/or psychiatric advanced directives with clients
- Therapeutic services include assessments, comprehensive case plans and reviews, individual and group therapy, as appropriate
- Work collaboratively with community partners
- Embed outreach and other services in alternate locations and with community providers to reduce barriers to access
- Provide services in-person or via telehealth platforms
- Gather and complete all CCBHC data metrics for homeless population served
