Housing Stability Coordinator
JOB SUMMARY
The Housing Stability Coordinator supports individuals experiencing housing instability, with a primary focus on Housing Visions' low-moderate income households and Medicaid members. This role involves conducting trauma-informed assessments and developing individualized service plans to increase self-sufficiency and promote independent living, along with screening for health-related social care needs through the NYS 1115 Medicaid Waiver. The Housing Stability Coordinator provides ongoing case management, advocacy, safety planning, and crisis intervention, as well as community-building activities and connections to community-based resources. The overall goal of CONNECT is to support residents in securing and maintaining safe, quality, affordable housing and fostering a supportive environment which reflects the mission, vision, and values of Housing Visions.
Responsibilities
- Conduct trauma-informed assessments and develop individualized service plans
- Screens for health-related social care needs through the NYS 1115 Medicaid Waiver
- Provide ongoing case management, advocacy, safety planning, and crisis intervention
- Community-building activities and connections to community-based resources
Requirements
- Bachelor's degree in Social Work, Psychology, Public Health, or related field
- Experience working with individuals experiencing housing instability
- Knowledge of trauma-informed care principles
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Proficient in Microsoft Office Suite
Qualifications
- Current New York State Driver’s License
- Valid New York State Vehicle Registration
- Passing background check
Skills
- Strong organizational and time management skills
- Excellent written and verbal communication skills
- Ability to work flexible hours including evenings and weekends
- Experience with case management software

Pay
Pay Range is $25-$27/hr.
Schedule
Monday – Friday 8:30 am - 5:00pm, with occasional night/weekend hours.
