About the role
The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement support. They also manage the team's schedule and ensure coordination of services around the clock.
Responsibilities
- Supervise, evaluate, and coordinate the interdisciplinary team
- Evaluate patient admissions and discharges for eligibility and appropriateness
- Serve as patient advocate and coordinator for social services and healthcare providers
- Maintain and document patient records accurately and timely
- Assure continuity of care from admission to discharge or transfer
- Facilitate interdisciplinary meetings to update and revise the plan of care
- Manage team schedule and provide on-call coverage
- Interview, select, train, supervise, and dismiss team staff
- Ensure staff competence and performance through regular evaluations and visits
- Oversee staff and volunteer schedules, ensuring equitable distribution of workload
- Review and approve payroll, ensuring accurate time and expense documentation
- Monitor clinical records and team activities to ensure compliance with standards
- Develop and implement performance improvement activities
- Complete and implement patient/family satisfaction surveys
- Participate in regulatory body surveys and requests
- Perform utilization reviews and cost containment
- Attend inservices, educational seminars, and workshops
- Develop and achieve professional growth goals
- Mentor newly hired Team Managers
Requirements
- Reliable transportation with appropriate license and insurance coverage
- Private telephone in home
- Knowledge of primary medical and nursing care principles, including oncology and palliative care
- Understanding of interdisciplinary health care team dynamics and the process of dying and bereavement
- Working knowledge of VITAS information system (Vx) and its management reports
- Two years of supervisory experience in a health care organization
- Bachelor's degree preferred
- Current and valid R.N. License to practice in the state where the VITAS program is located
