Team Manager Home Care

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement, and ensuring these services align with the interdisciplinary plan of care.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team
• Evaluate and document patient admissions and discharges
• Serve as patient advocate and coordinator for external healthcare providers
• Maintain and update patient records
• Facilitate interdisciplinary meetings and plan of care updates
• Manage team schedule and provide on-call support
• Interview, select, train, supervise, and dismiss team staff
• Monitor and approve payroll, ensuring accurate time and expenses
• Ensure compliance with regulatory standards and conduct quality improvement activities
• Assure patient/family satisfaction through proactive surveys and resolution of grievances
• Participate in customer service training and inservices
• Perform utilization reviews and cost containment activities
• Attend professional development opportunities and achieve personal growth goals

Requirements

• Reliable transportation with appropriate license and insurance
• Private telephone in home
• Knowledge of primary medical and nursing care principles
• Understanding of interdisciplinary health care dynamics and end-of-life processes
• Working knowledge of VITAS information systems and management reports
• Two years supervisory experience in a health care setting
• Bachelor's degree preferred
• Current R.N. license in the state where the VITAS program is located

• Experience in managing interdisciplinary teams
• Ability to maintain accurate and complete patient records
• Effective communication and problem-solving skills
• Strong organizational and time management abilities
• Proficient in using VITAS information systems
