About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement, and ensuring these services align with the interdisciplinary plan of care.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team, ensuring continuity of care from admission to discharge or transfer to bereavement.
• Serve as patient advocate and coordinator for social service and health care providers in the community.
• Maintain patient records from intake through discharge or transfer to bereavement, ensuring eligibility and appropriateness criteria are met.
• Collaborate with the team physician and attending physician on palliative care measures for pain and symptom management.
• Review and approve payroll, ensuring accurate time, mileage, and additional expenses.
• Manage the team schedule and coordinate services 24/7, facilitating interdisciplinary meetings and updating the plan of care.
• Assure staff competence and performance through field visits, evaluations, and in-services.
• Oversee staff and volunteer schedules, ensuring equitable distribution of workload and meeting productivity expectations.
• Perform substantive chart reviews to ensure quality care is being delivered, and participate in outcomes management and annual program review.
• Participate in professional development activities, achieving professional growth goals and mentoring newly hired Team Managers.

Qualifications

• Reliable transportation with appropriate license and insurance coverage.
• Two years successful supervisory experience or equivalent in a health care organization.
• Bachelor's degree preferred.
• Current and valid R.N. License to practice in the state where the VITAS program is located.
