Team Manager Home Care

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services, including volunteer and bereavement support. They ensure services align with the interdisciplinary plan of care and maintain accurate documentation.

Responsibilities

• Supervise, evaluate, and coordinate interdisciplinary team members
• Evaluate and approve patient admissions, assessments, and discharges
• Collaborate with physicians on palliative care measures
• Ensure complete and accurate patient records for recertification
• Plan and manage discharge processes
• Manage team schedules and provide 24/7 coverage
• Facilitate interdisciplinary meetings and update care plans
• Participate in on-call rotations

Requirements

• Reliable transportation with appropriate license and insurance
• Knowledge of primary medical and nursing care principles
• Understanding of interdisciplinary health care dynamics
• Basic knowledge of end-of-life care and bereavement
• Experience with VITAS information systems
• Two years supervisory experience in a healthcare setting

Qualifications

• Current RN license
• Bachelor's degree preferred
• Knowledge of VITAS information system
• Working knowledge of oncology and palliative care
• Ability to work independently and manage multiple tasks
