About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services and ensuring seamless integration of interdisciplinary care plans.

Responsibilities

  • Supervise, evaluate, and coordinate the Patient Care Team, including volunteers and bereavement services.
  • Evaluate patient admissions, assessments, and discharges to ensure eligibility and appropriateness.
  • Collaborate with physicians on palliative care measures and discharge planning.
  • Maintain and document patient records accurately and in compliance with regulatory standards.
  • Manage team schedules and ensure 24/7 availability for all patients.
  • Facilitate interdisciplinary meetings and update care plans.
  • Participate in on-call rotations and provide customer service.
  • Oversee staff supervision, training, and performance evaluations.
  • Ensure quality improvement initiatives and participate in regulatory surveys.
  • Monitor and control costs related to patient care services.
  • Attend professional development opportunities and achieve personal growth goals.

Requirements

  • Reliable transportation with appropriate license and insurance.
  • Knowledge of primary medical and nursing care principles, oncology, and palliative care.
  • Understanding of interdisciplinary health care dynamics and the process of dying and bereavement.
  • Working knowledge of VITAS information systems and management reports.
  • Two years of supervisory experience in a healthcare setting.
  • Bachelor's degree preferred.
  • Current R.N. license in the state where the VITAS program is located.

Qualifications

  • Basic understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Knowledge of VITAS information system (Vx).
  • Ability to utilize management reports effectively.
  • Experience in managing team schedules and ensuring 24/7 availability.
  • Skills in customer service, problem-solving, and conflict resolution.
  • Proficiency in using communication tools to address patient concerns and grievances.
