About the role
The Team Manager leads the Patient Care Team, overseeing all patient care services and ensuring seamless integration of interdisciplinary care plans.
Responsibilities
- Supervise, evaluate, and coordinate the Patient Care Team, including volunteers and bereavement services.
- Evaluate patient admissions, assessments, and discharges to ensure eligibility and appropriateness.
- Collaborate with physicians on palliative care measures and discharge planning.
- Maintain and document patient records accurately and in compliance with regulatory standards.
- Manage team schedules and ensure 24/7 availability for all patients.
- Facilitate interdisciplinary meetings and update care plans.
- Participate in on-call rotations and provide customer service.
- Oversee staff supervision, training, and performance evaluations.
- Ensure quality improvement initiatives and participate in regulatory surveys.
- Monitor and control costs related to patient care services.
- Attend professional development opportunities and achieve personal growth goals.
Requirements
- Reliable transportation with appropriate license and insurance.
- Knowledge of primary medical and nursing care principles, oncology, and palliative care.
- Understanding of interdisciplinary health care dynamics and the process of dying and bereavement.
- Working knowledge of VITAS information systems and management reports.
- Two years of supervisory experience in a healthcare setting.
- Bachelor's degree preferred.
- Current R.N. license in the state where the VITAS program is located.
Qualifications
- Basic understanding of interpersonal dynamics within an interdisciplinary health care team.
- Knowledge of VITAS information system (Vx).
- Ability to utilize management reports effectively.
- Experience in managing team schedules and ensuring 24/7 availability.
- Skills in customer service, problem-solving, and conflict resolution.
- Proficiency in using communication tools to address patient concerns and grievances.
