Team Manager Home Care

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement, and ensuring these services align with the interdisciplinary plan of care.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team, ensuring continuity of care from admission to discharge or transfer to bereavement.
• Serve as patient advocate and coordinator for social service and health care providers.
• Maintain patient records and support recertification decisions.
• Assure effective discharge planning for imminent discharges.
• Manage team schedule and services 24/7.
• Facilitate interdisciplinary meetings and update care plans.
• Participate in on-call rotation.

Requirements

• Reliable transportation with appropriate license and insurance.
• Private telephone in home.
• Knowledge of primary medical and nursing care, oncologic nursing, and palliative care.
• Understanding of interdisciplinary health care team dynamics and the process of dying and bereavement.
• Working knowledge of VITAS information system and management reports.
• Two years of supervisory experience or equivalent in a health care setting.

Qualifications

• Bachelor's degree preferred.
• Current R.N. license in the state where the VITAS program is located.
