Team Manager Home Care

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement, and ensuring these services align with the interdisciplinary plan of care.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team
• Evaluate and approve patient admissions and discharges
• Serve as patient advocate and coordinator for social services and healthcare providers
• Maintain and document patient records accurately
• Assure continuity of care from admission to discharge or transfer
• Facilitate interdisciplinary meetings and update care plans
• Manage team schedule and provide on-call support
• Interview, select, train, supervise, and dismiss team staff
• Oversee staff and volunteer schedules, ensuring equitable distribution of workload
• Review and approve payroll, ensuring accurate time and expenses
• Monitor and improve clinical records and team activities
• Perform utilization reviews and participate in quality improvement initiatives
• Address customer service issues and resolve grievances promptly
• Participate in professional development activities and mentor new Team Managers

Qualifications

• Reliable transportation with appropriate license and insurance
• Two years successful supervisory experience or equivalent in a health care organization
• Bachelor's degree preferred
• Current and valid RN license to practice in the state where the VITAS program is located
