Support Services Specialist for the Homeless

About the Role

Coordinate resources and care for persons enrolled in the program. Manage the provision of individual client services, ensuring each client's needs are met. Intervene and problem-solve issues if outcomes, goals, and service delivery are not being met according to program goals and established timelines.

Responsibilities

• Develop clients' initial intake, progress, and discharge reports; track and document the status of persons served.
• Schedule and facilitate interdisciplinary case conferences with all community partners, funders, and family members.
• Arrange for the provision of necessary equipment to enable the achievement of agreed-upon functional goals.
• Participate in Program Evaluation and Quality Assurance activities.
• Maintain the highest standard of professional conduct regarding confidential information; share information only when the recipient's right to access is clearly established and it is in the best interest of the person served.
• Adhere to and participate in the Company's mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs/practices.
• Attend, participate in, and/or conduct internal staff development programs; obtain continuing education as required by Company policy and regulations.
• Perform other duties as assigned.

Requirements

• Prior experience in a 24-hour care facility preferred.
• Bachelor's degree or equivalent experience preferred.
• Demonstrated crisis management ability.
• Effective and professional interpersonal, verbal, and written communication skills.
• Strong computer software (Microsoft Office) and data entry skills.
• Able to work independently and as part of a team.
• Organized and able to function under minimal supervision.
• Reliable transportation and ability to travel to/from multiple assignments/locations on a daily basis.
