Peer Support Specialist (Homeless Services)
About the role
Family Health Centers of San Diego (FHCSD) has been providing caring, affordable, high-quality health care since 1968. We are one of the top 10 largest federally qualified health centers (FQHCs) in the country, operating over 90 sites across San Diego County. Our services include adult care, chronic disease management, pediatrics, comprehensive women's care, dental, vision, case management, physical rehabilitation, speech therapy, infectious diseases, behavioral health, substance use counseling, and a variety of specialty services. We also offer supportive services to those experiencing homelessness.
Responsibilities
- Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards.
- Engages patients in meeting their healthcare needs and cultivates patient connectedness with the clinic and community.
- Participates in care coordination.
- Leads and/or co-leads peer recovery groups.
- Provides engagement, assistance, and support to patients, encouraging healthy activities and compliance.
- Utilizes supervision and reports any high risk situations.
- Performs other duties as assigned.
Requirements
- 2 years personal experience as a mental health services consumer.
- Bilingual in English/Spanish required.
- Completion of Peer Employment Training.
- High school diploma or GED equivalent, or equivalent combination of education and experience.
- Valid California driver’s license and proof of minimum levels of car insurance as required under California law.
- Ability to maintain good interpersonal interactions with clients and co-workers.
- Intermediate interpersonal and customer service skills.
- Intermediate written and verbal communication skills, including ability to communicate effectively with patients and family members who speak English as a second language.
