This position collaborates with Senior TLC care providers and the Kintegra Pharmacy staff, providing oversight and advisement regarding pharmaceutical treatment for Senior TLC participants. The position reports directly to the Senior TLC Medical Director.
Qualifications
- Bachelor of Science in Pharmacy or Pharm. D. degree
- Three years of clinical pharmacy experience with one year of experience with a frail or elderly population
- Unrestricted Pharmacy license in the state of North Carolina
- Ability to learn, teach, and research; ability to solve problems and think analytically
- Skills necessary to treat geriatric patients and work in a team environment
- Knowledge and skills necessary to treat patients within the scope of authority
- Must meet a standardized set of PACE-specific organization competencies prior to working independently
- Embraces and supports the team-based PACE Model
- Familiarity with pharmacy software programs necessary to execute daily tasks
- Prior FQHC or public health experience preferred
Responsibilities
- Collaborates with physicians, nurses, and other healthcare professionals, providing education, communication, and support for healthcare staff, participants, and families
- Collaborates with Senior TLC Providers in medication review and management during assessments and ongoing as required
- Facilitates monthly Polypharmacy meetings with Providers
- Participates in the Senior TLC Falls Committee
- Co-facilitates Pharmacy Committee with Kintegra Lead Pharmacist
- Monitors drug utilization patterns for potential fraud and abuse, including TGH data analysis (e.g., e-prescribed prescriptions)
- Conducts data analysis of pricing data to detect outliers and inappropriate changes in formularies
- Leads the Part D Risk Assessment process in collaboration with the Senior TLC Director of Quality and Corporate Compliance
- Participates in participant care planning, including the implementation of SMART goals/interventions for the participant’s plan of care, and enters all care plan information in a timely manner as per organizational protocols
- Supports the Interdisciplinary Team and promotes unity among the team while interacting with team members, co-workers, and/or participants
- Adheres to the “do no harm” team contract statement and emphasizes teamwork and collaboration in all interactions
- Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community, its vision to be the preferred provider of individualized care for seniors, and its values of respect, integrity, accountability, compatible goals, and compassionate care
