Clinical Pharmacist

Responsibilities

• Determine the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing patients in accordance with Option Care policies.
• Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients, including making recommendations for modifications in prescribed medication therapy or monitoring parameters.
• Collect and organize patient-specific information to prevent, detect, and resolve medication-related problems, including completing nutritional assessments and making appropriate recommendations.
• Modify pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information, demonstrating competency in pharmacokinetic principles and assessing patient laboratory values.
• Provide medication-use education to Option Care staff, other professionals, patients, and/or their caregivers using effective patient education techniques.
• Ensure continuity of pharmaceutical care to and from the home and other patient-care settings by systematically communicating pertinent patient information.
• Prepare and dispense medications using appropriate techniques, adhering to safety and quality assurance practices, and complying with state/federal laws and USP standards, including DEA controlled substance regulations.
• Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment, including programming, maintaining, and advising others on their use.
• Participate in assessing, managing, and reporting medication-related incidents and equipment errors/failures, and complete QAR documents according to Option Care policy.
• Provide concise, applicable, and timely responses to drug information requests from healthcare providers and home care patients, formulating responses based on literature analysis.
• Attend educational seminars or participate in other CEU opportunities to promote professional competency and comply with state-mandated CEU requirements.
• Manage the use of investigational drug products according to established regulations and Option Care policies and procedures.
• Utilize pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring the accuracy of their work.
• Maintain confidentiality of patient and proprietary information, observing legal and ethical guidelines.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures.
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
  • Completion of an ASHP accredited Pharmacy Residency Program.
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Experience providing customer service to internal and external customers, including meeting quality standards for services and evaluation of customer satisfaction.
• Basic level skills in Microsoft Word, Excel, and PowerPoint.
• Willing to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in the development, training, and assignment of work/projects.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range is $107,330.80 - $178,899.67.
