About the role

The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, professional standards, and regulatory requirements. This entry-level position coordinates patient services with the Nursing Department, monitors patient progress, and educates patients, caregivers, and professionals on appropriate medication use.

Responsibilities

• Determines the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing their medical condition and prescribed medication therapy.
• Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for home care patients, including modifications to prescribed medication therapy or monitoring parameters.
• Completes nutritional assessments and makes appropriate recommendations.
• Modifies pharmacy care plans based on evaluation of monitoring data and other patient-specific information, demonstrating competence in pharmacokinetic principles and laboratory value assessment.
• Provides medication-use education to Option Care staff, other professionals, patients, and caregivers using effective patient education techniques.
• Ensures continuity of pharmaceutical care to and from the home and other patient-care settings through systematic communication of pertinent patient information.
• Prepares and dispenses medications using appropriate techniques, ensuring accuracy, stability, and adherence to safety and quality assurance practices.
• Complies with state/federal laws, USP standards, and DEA controlled substance regulations, including managing perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
• Manages the use, maintenance, and troubleshooting of medication administration and related equipment, including assessing, managing, and reporting medication-related incidents or equipment failures.
• Provides concise, applicable, and timely responses to drug information requests from healthcare providers and patients, based on literature analysis.
• Attends educational seminars or participates in continuing education to maintain professional competency and comply with state-mandated requirements.
• Manages the use of investigational drug products according to established regulations and Option Care policies.
• Effectively utilizes pharmacy support personnel, prioritizing workload and organizing workflow to ensure timely completion of tasks.
• Maintains confidentiality of patient and proprietary information in accordance with legal and ethical guidelines.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures, or
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), or
  • Completion of an ASHP accredited Pharmacy Residency Program, or
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Experience providing customer service to internal and external customers.
• Basic proficiency in Microsoft Word, Excel, and PowerPoint.
• Willing to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay range is $92,004.26 - $153,332.93.
