Responsibilities

• Determines the suitability of individual patients for home care by collecting and organizing patient-specific information and assessing patients in accordance with Option Care policies.
• Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans for home care patients, including making modifications to prescribed medication therapy or monitoring parameters.
• Completes nutritional assessments of patients and makes appropriate recommendations.
• Modifies pharmacy care plans based on evaluation of monitoring data and other pertinent patient-specific information, including use of pharmacokinetic principles and assessment of laboratory values.
• Provides medication-use education to Option Care staff, other professionals, patients, and/or their caregivers using effective patient education techniques.
• Ensures continuity of pharmaceutical care to and from the home and other patient-care settings by systematically communicating pertinent patient information.
• Prepares and dispenses medications using appropriate techniques, ensuring medications are appropriately concentrated, stable, and stored, and adhering to safety and quality assurance practices.
• Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment, including programming, maintenance, and advising others on use.
• Participates in assessing, managing, and reporting medication-related incidents and equipment errors or failures, and completes QAR documents according to Option Care policy.
• Monitors and reports discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion.
• Provides concise, applicable, and timely responses to drug information requests from healthcare providers and patients, formulating responses based on literature analysis.
• Attends educational seminars or participates in continuing education opportunities to maintain professional competency and comply with state-mandated requirements.
• Manages the use of investigational drug products according to established regulations and Option Care policies.
• Utilizes pharmacy support personnel effectively by prioritizing workload, organizing workflow, and ensuring accuracy of their work.
• Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines.
• Complies with accreditation, legal, regulatory, and safety requirements in all activities, including state/federal laws, USP standards, and DEA controlled substance regulations.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures.
  • Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
  • Completion of an ASHP accredited Pharmacy Residency Program.
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Experience providing customer service to internal and external customers, including meeting quality standards for services and evaluating customer satisfaction.
• Basic level skills in Microsoft Word, Excel, and PowerPoint.
• Willing to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean room settings.
• At least 1 year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects.

Pay

Pay Range is $107,330.80 - $178,899.67. Salary is determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
