Responsibilities

• Affords the physician in evaluating the patient’s level of function.
• Affords the assessment of the patient’s home environment and identification of equipment needs related to speech/language function.
• Affords the identification of functional speech deficits and establishment of goals and plan of care to improve patient’s function, revising as necessary.
• Affords the performance and completion of recertifications, resumption-of-care, and discharge.
• Affords the Comprehensive OASIS assessments and associated paperwork in accordance with agency policy.
• Affords the instruction of patient and family members, as well as other health team members, in methods of assisting the patient in improving communication skills.
• Affords the recommendation of communication devices/aids as indicated.
• Affords the periodic re-evaluation and upgrading of the patient’s program to maximize functional improvement.
• Affords the recording and reporting to the patient’s physician the patient’s progress in treatment and any changes in the patient’s condition and plan of care.
• Affords the conference with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
• Affords the performance and submission of required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
• Affords the return of all patients visit notes in on the next scheduled workday, not to exceed forty-eight (48) hours.
• Affords the maintenance of necessary statistical and billing data.
• Affords the keeping abreast of changes in speech pathology practice and theory, contributing to the field through research, presentations, and in-services.
• Affords the attendance and contribution to in-services, clinical records reviews, meetings required to ensure coordinated and comprehensive plans of care for the patients of the agency.

Qualifications

• Primary series of the COVID-19 vaccine required-NE Only
• A Master’s Degree in Speech Pathology from a Department of Speech Pathology at an accredited college approved by the American Board of Examiners in Speech Pathology and Audiology of the American Speech and Hearing Association.
• Licensed in the State of practice by the StateBoard of Healing Arts to practice as a Speech Pathologist.
• Certificate of Clinical competence in speech pathology or audiology.
• Must possess and maintain proof of current CPR training.
