Speech Therapist - SLP

About the Role

The Speech Therapist in Home Health assesses and treats patients with speech, language, swallowing, and cognitive impairments. They develop and implement individualized treatment plans, monitor patient progress, and collaborate with healthcare team members to enhance patient well-being.

Responsibilities

• Provide speech therapy services within the scope of practice and in accordance with the plan of care
• Evaluate patient function using diagnostic tests and assist in developing therapy plans with physicians
• Treat patients to improve communication and swallowing functions
• Observe, document, and report patient responses and condition changes
• Educate patients, families, caregivers, and team members on therapy techniques and activities

Requirements

• Licensed in Speech Therapy in the state of residence
• Current CPR certification
• Valid driver’s license, reliable transportation, or access to public transit
• Availability for weekend rotations
