Responsibilities

The Speech Language Pathologist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. This may include a patient's private residence, assisted living facility, or independent living facility. Powerback Rehabilitation To You allows evaluation and treatment in the patient's home, the most functional environment.

• Organize and provide speech-language services to facilitate rehabilitation, directing patient participation in tasks to restore, reinforce, and enhance performance.
• Facilitate learning of skills and functions essential for communication and swallowing, diminishing or remediating disorders or deficits.
• Direct activities to promote and maintain health within the scope of practice and consistent with the Code of Ethics of the profession.
• Supervise CFYs, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
• Attend and contribute to patient care, staffing conferences, and related meetings.
• Make referrals to other disciplines or agencies to address comprehensive patient needs.
• Consult with rehabilitation team members and physicians to select appropriate therapeutic regimens based on patient needs and capabilities.
• Adapt programs and care plans according to individual patient needs.
• Promote a safe environment and eliminate fire and safety hazards.
• Order, clean, maintain, and report the need for repair of supplies and equipment.
• Prioritize patient service, ensuring residents and families receive high-quality care in a compassionate atmosphere.
• Screen, examine, and evaluate patients, including history, systems review, and application of tests and measures to establish a diagnosis and plan of care.
• Develop treatment goals and methods in collaboration with patients and caregivers, implementing the speech-language pathology treatment plan.
• Provide therapeutic techniques in areas such as cognitive-communication, speech intelligibility, oral motor skills, swallowing disorders/dysphagia, perceptual abilities, orientation, memory, pragmatics, psychosocial expression, functional communication, and case management in home and community settings.
• Coordinate the acquisition of adaptive communication devices and instruct patients in their use.
• Facilitate identification of hearing deficits and refer for assessment, diagnosis, rehabilitation, and/or personal amplification.
• Select appropriate activities suited to the individual's physical and mental capacity, intelligence level, and interest.
• Instruct patients in skills and techniques required for therapeutic activities and evaluate progress, attitudes, and behavior.
• Perform routine reexamination to modify or progress the treatment plan as needed.
• Coordinate care in the home and community with interdisciplinary team members in collaboration with the referring physician.
• Perform other related duties as required.

Qualifications

• Valid Speech Language Pathologist license in the state(s) of practice or proof of license eligibility.
• Valid driver's license or reliable, independent transportation to and from patients' residences.

Pay

USD $48.00 - USD $61.00 per hour.
