About the Role

The Speech Therapist in Home Health assesses, evaluates, and treats patients with speech, language, swallowing, and cognitive impairments to improve communication and overall well-being. They develop and implement personalized treatment plans, monitor patient progress, and collaborate with healthcare team members to ensure comprehensive care.

Responsibilities

• Provide speech therapy services within legal and professional guidelines, aligned with the patient's care plan
• Evaluate patient function using diagnostic tests and assist in creating therapy plans with physicians
• Treat patients to enhance communication abilities based on individual conditions
• Observe, document, and report patient responses and changes to supervising staff
• Educate patients, families, caregivers, and team members on speech therapy techniques and participation

Requirements

• Licensed in Speech Therapy in the state of residence
• Current CPR certification
• Valid driver's license, reliable transportation, or access to public transit
