Speech Therapist - Mobile Outpatient

About the role

We are seeking a Speech Language Pathologist to provide outpatient rehabilitation services primarily in patients' homes, including private residences and assisted or independent living facilities. Evaluate and treat communication and swallowing disorders, focusing on functional, patient-centered care in various settings.

Responsibilities

• Evaluate and treat communication and swallowing disorders
• Supervise CFYs, students, and support personnel as applicable, ensuring adherence to licensure standards
• Develop and modify individualized treatment plans, document progress, and collaborate with interdisciplinary teams and physicians
• Cook up referrals, order supplies, and maintain equipment to support patient needs
• Promote a safe environment, advocate for patient rights, and ensure high-quality, compassionate care
• Perform assessments, implement therapeutic techniques across areas such as cognition, speech, swallowing, and social communication
• Instruct and assist patients with adaptive devices, hearing assessments, and relevant techniques to enhance communication
• Maintain timely documentation, participate in meetings, and support a flexible work schedule in diverse care environments
