About the role

As a Speech Therapist at VNA of Florida, you will ensure the delivery of quality speech therapy services and provide case management for patients through comprehensive assessments and coordination of agency services. You will work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes.

Responsibilities

• Ensure delivery of quality speech therapy services
• Provide case management for patients through comprehensive assessments and coordination of agency services
• Work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes
• Complete physical assessments and determine any patient home care needs
• Work with the care team for each patient while using VNA of Florida's case management model

Requirements

• Bachelor’s degree in Speech-Language Pathology or related field
• Current license to practice as a Speech-Language Pathologist in Florida
• Experience in home health or related field preferred
