About the role

TGH Home Care powered by VNA of Florida seeks to provide high-quality home care services. This position ensures the delivery of quality speech therapy services and manages cases for patients in their homes.

Responsibilities

• Ensure delivery of quality speech therapy services
• Provide case management for patients through comprehensive assessments and coordination of agency services
• Work with family members and local healthcare professionals to implement services to meet the needs of patients and families in their homes
• Complete physical assessments and determine any patient home care needs
• Work with the care team for each patient while using VNA of Florida's case management model

Requirements

• Bachelor’s degree in Speech-Language Pathology or related field
• Current license to practice as a Speech-Language Pathologist in Florida
• Experience in providing speech therapy services in a home health setting
• Excellent communication and interpersonal skills
• Ability to travel within Pasco County
