About the role
Provides speech therapy services to patients with speech, language, or swallowing disorders in a home health or hospice setting.
Responsibilities
- Assess and document patient data related to speech, care needs, interventions, and outcomes
- Monitor changes in physical and emotional parameters
- Review and adjust treatment plans, and develop new programs as needed
- Maintain accurate documentation in compliance with legal, organizational, and regulatory standards
- Instruct patients on monitoring speech and practicing new skills
- Perform additional related duties as assigned
Qualifications
- Master's Degree or foreign equivalent in speech-language pathology
- State licensure as a Speech Pathologist
- CPR or BLS certification from an approved provider
- Valid driver’s license and age of at least 21
Preferred: ASHA Certificate of Clinical Competence (CCC)
