About the role
Supporting individuals in maintaining independence and navigating long-term care services within a social services capacity.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Conduct regular contacts and visits
- Assist with IRIS eligibility
- Connect with local agencies
- Document interactions promptly
- Conduct annual functional screens
- Collaborate with a team while working independently
Qualifications
- Bachelor’s degree in social work, psychology, or a related field with at least one year of experience working with seniors or individuals with disabilities
- OR four or more years of relevant social services experience
- Bilingual skills are a plus
- Must possess a valid driver’s license, reliable transportation, and meet auto insurance requirements
