About the role

Supporting individuals in maintaining independence and navigating long-term care services within a social services capacity.

Responsibilities

  • Build trusting relationships with participants
  • Develop IRIS Support and Services Plans
  • Conduct regular contacts and visits
  • Assist with IRIS eligibility
  • Connect with local agencies
  • Document interactions promptly
  • Conduct annual functional screens
  • Collaborate with a team while working independently

Qualifications

  • Bachelor’s degree in social work, psychology, or a related field with at least one year of experience working with seniors or individuals with disabilities
  • OR four or more years of relevant social services experience
  • Bilingual skills are a plus
  • Must possess a valid driver’s license, reliable transportation, and meet auto insurance requirements
