About the role

Join us as an IRIS Consultant, supporting individuals to live independently and manage their own care through the IRIS (Include, Respect, I Self-Direct) program. In this social services role, you will build trust with participants, develop IRIS Support and Services Plans, and maintain regular contact via monthly and quarterly visits. You will assist clients in understanding IRIS eligibility, connect with local agencies to expand services, and document all interactions promptly. The role involves traveling within several counties and may include conducting annual functional screens.

Requirements

• Bachelor's degree in social work or related field with experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
• Valid driver's license, reliable transportation, and a clean driving record
