About the role
Join as an IRIS Consultant to support individuals in maintaining independence and accessing long-term care services.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting quarterly visits
- Assisting participants with IRIS eligibility and connecting with local agencies
- Documenting all interactions promptly
- Conducting annual Long-Term Care Functional Screens (if applicable)
- Working independently within a collaborative team
Qualifications
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Multilingual skills are a plus
- Strong commitment to participant autonomy and empathy
- Proficiency with smartphones, tablets, and computers
- Knowledge of social behavior, resources, and community systems
- Valid driver’s license and reliable transportation
