About the role
Join as an IRIS Consultant to support individuals in living independently through the IRIS (Include, Respect, I Self-Direct) program.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Assist participants with IRIS eligibility and connect them with local agencies
- Document all interactions promptly
- Conduct annual Long-Term Care Functional Screens
- Work independently with team collaboration
Qualifications
- Bachelor’s degree in social work, psychology, or a related field with at least one year of supervised experience with seniors or individuals with disabilities
- OR four or more years of relevant social services experience
- OR current IRIS Consultant in good standing
- Bilingual skills are a plus
- Strong commitment to participant autonomy and respectful service
- Comfortable with technology and community resources
- Willing to serve diverse populations with empathy
Requirements
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
