About the role
The IRIS Consultant supports individuals in maintaining independence and self-direction within the IRIS (Include, Respect, I Self-Direct) program.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Conduct regular contacts and visits
- Assist with eligibility determination
- Connect participants to community resources
- Complete required documentation
- Possibly conduct annual functional screens
- Collaborate with a supportive team while working independently
Qualifications
- Bachelor’s degree in a related field with at least one year of supervised experience working with seniors or individuals with disabilities
- OR four or more years of direct social service experience
Requirements
- Valid driver’s license
- Reliable transportation and ability to meet auto insurance standards
Skills
- Bilingual skills (advantageous)
- Familiarity with technology (advantageous)
