Social Services Consultant - Hybrid, Full Benefits

Role Overview

Support individuals in maintaining independence and self-direction through IRIS (Include, Respect, I Self-Direct). Build trusting relationships, develop IRIS Support and Services Plans, and assist participants in understanding and maintaining IRIS eligibility.

Responsibilities

• Conduct monthly contacts and quarterly visits
• Connect with local agencies
• Document interactions promptly
• May perform annual functional screens
• Work independently with team collaboration

Qualifications

• Bachelor's degree in social work, psychology, or related fields with at least one year of experience with seniors/people with disabilities
• OR 4+ years of direct social services
• Bilingual skills are a plus
• Must have a valid driver's license, reliable transportation, and a commitment to respectful service
