About the role

Join as an IRIS Consultant to support individuals in maintaining independence and self-directed care through the IRIS program.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining regular contact and conducting visits
  • Assisting with IRIS eligibility and connecting with local agencies
  • Documenting all interactions promptly
  • Potentially conducting annual functional screens
  • Collaborating with a team while working independently

Qualifications

  • Bachelor's degree in social work, psychology, or related fields plus at least one year of supervised experience with seniors or individuals with disabilities, or four+ years of direct social services experience
  • Current IRIS Consultant status with DHS exemption is a plus
  • Bilingual skills in Spanish, Hmong, Arabic, Russian, or others are advantageous
  • Strong commitment to participant autonomy, empathy, and respect
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior and community resources
  • Valid driver's license and reliable transportation
