About the role
As an IRIS Consultant, you will support individuals in maintaining independence and self-direction through the IRIS program. Your responsibilities include building trusting relationships, developing IRIS Support and Services Plans, conducting regular contacts and visits, and assisting participants with eligibility and service options. You will collaborate with local agencies, document interactions promptly, and may conduct annual functional screens. The role requires working independently within a social services capacity, with travel across designated counties.
Responsibilities
- Build trusting relationships with participants
- Develop and maintain IRIS Support and Services Plans
- Conduct regular contacts, visits, and annual assessments
- Assist with eligibility and connect participants to local resources
- Document all interactions within required systems
Qualifications
A Bachelor's degree in social work, psychology, or related fields with at least one year of supervised experience working with seniors or individuals with disabilities, or four or more years of relevant social services experience. Bilingual abilities are a plus.
