About the role
Join as an IRIS Consultant and support individuals in living independently through the IRIS (Include, Respect, I Self-Direct) program. This social services role involves building trusting relationships, developing IRIS Support and Services Plans, maintaining regular contact, and helping participants understand and sustain IRIS eligibility. You will connect with local agencies to broaden service options, document all contacts promptly, and may conduct annual functional screens. The position requires working independently while collaborating with a supportive team.
Requirements
- Valid driver’s license and reliable transportation
- Satisfactory driving record and auto insurance compliance
Qualifications
- A Bachelor’s Degree in social work, psychology, human services, counseling, nursing, special education, or a related field with at least one year of supervised experience working with seniors or individuals with disabilities
- OR four or more years of direct social services experience
- OR current IRIS Consultant in good standing with DHS exemption
- Bilingual skills are a plus
- Strong commitment to participant autonomy
- Comfort with technology and knowledge of community resources
- Willingness to serve diverse populations with respect and empathy
