About the role

An IRIS Consultant supports individuals in maintaining independence and self-direction within the IRIS (Include, Respect, I Self-Direct) program. Based in Milwaukee with travel to surrounding counties, you will build trusting relationships, develop personalized IRIS Support and Services Plans, and assist participants in understanding and maintaining their eligibility. You will also connect clients with local agencies, document contacts promptly, and may conduct annual functional screens. The role requires independent work paired with team collaboration.

Responsibilities

  • Build and maintain participant relationships
  • Develop and review IRIS Support and Services Plans
  • Maintain regular contact through visits and calls
  • Assist participants in understanding IRIS eligibility
  • Coordinate with local agencies to expand services
  • Document all interactions within required systems
  • Conduct annual functional assessments (if applicable)

Qualifications

  • A Bachelor’s degree in social work, psychology, human services, counseling, nursing, or a related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
  • Bilingual skills are a plus
  • Demonstrated strong commitment to participant autonomy
  • Comfort with technology and knowledge of community resources and human behavior
  • A valid driver’s license and reliable transportation
