About the role
Join as an IRIS Consultant and support individuals in maintaining independence and self-direction through the IRIS program.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain regular contact and conduct visits
- Help participants understand and maintain IRIS eligibility
- Connect with local agencies to expand service options
- Document all interactions promptly
- Potentially conduct annual Long-Term Care Functional Screens
- Work independently within a collaborative team environment
Qualifications & Skills
- Bachelor’s degree in social work, psychology, human services, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Bilingual abilities in Spanish, Hmong, Arabic, Russian, or other languages are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Willingness to serve diverse populations with empathy
