About the role
We are seeking an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.
Responsibilities
- Build trusting relationships with participants
- Develop IRIS Support and Services Plans
- Maintain ongoing contact and conduct visits
- Assist with IRIS eligibility and connecting with local agencies
- Document interactions in required systems promptly
- Conduct annual Long-Term Care Functional Screens (as needed)
- Collaborate with a supportive team while working independently
Qualifications & Skills
- Bachelor’s degree in social work, psychology, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing
- Strong commitment to participant autonomy and meaningful living
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interaction, and community resources
- Willingness to serve diverse populations with empathy and respect
- Valid driver’s license, reliable transportation, and satisfactory driving record
