About the role
IRIS Consultant supporting individuals to live independently through the IRIS program.
Responsibilities
- Build trusting relationships with participants
- Develop and maintain IRIS Support and Services Plans
- Conduct monthly contacts, quarterly visits, and annual functional screens
- Assist participants in understanding and maintaining IRIS eligibility
- Connect with local agencies and ADRCs to expand service options
- Document all contacts and visits promptly
- Support participant autonomy and navigate long-term care services
Qualifications & Skills
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field, or equivalent experience
- At least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
- Willingness to serve diverse populations with empathy and respect
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interaction, and community resources
- Valid driver’s license, reliable transportation, and meeting auto insurance requirements
- Bilingual skills are a plus
