Social Services Consultant - Hybrid, Full Benefits

About the role

Join our team as an IRIS Consultant and support individuals in maintaining independence and self-direction through the IRIS program.

Responsibilities

• Building trusting relationships with participants
• Developing IRIS Support and Services Plans
• Maintaining regular contact and conducting visits
• Helping participants understand and retain IRIS eligibility
• Connecting with local agencies to expand service options
• Documenting all contacts and visits promptly
• Conducting annual Long-Term Care Functional Screens (as needed)
• Collaborating with a supportive team while working independently

Qualifications

• Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field with at least one year of supervised experience working with seniors or individuals with disabilities
• OR 4+ years of direct social services experience with similar populations
• OR current IRIS Consultant in good standing with DHS exemption
• Multilingual skills are a plus
• Strong commitment to participant autonomy and respectful service
• Proficiency with smartphones, tablets, and computer systems
• Knowledge of human behavior, social interactions, and community resources
• Willingness to serve diverse populations with empathy

Requirements

• Valid driver’s license and reliable transportation
• Satisfactory driving record and auto insurance compliance
