About the role

Join a social services team as an IRIS Consultant, supporting individuals in maintaining independence, health, and safety through IRIS (Include, Respect, I Self-Direct). You will build trusting relationships, assist in developing IRIS Support and Services Plans, and conduct regular contact and visits. Your role includes helping participants understand IRIS eligibility, connecting with local agencies, and documenting interactions promptly. You may also conduct annual functional screens. This position requires working independently while collaborating with a supportive team.

Qualifications

  • Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field with at least one year of supervised experience with seniors or disabled populations;
  • OR 4+ years of direct social service experience;
  • OR current IRIS Consultant in good standing with DHS exemption.

Multilingual skills (e.g., Spanish, Hmong, Arabic, Russian) are a plus. Strong commitment to participant autonomy, proficient in technology, and knowledgeable about community resources.

Requirements

  • Valid driver’s license with reliable transportation and satisfactory driving record.
